Affirmative Therapy Collective (“ATC,” “we,” “us”) is the practice name of Affirmative Therapy LCSW PLLC. This Privacy Policy explains how we handle information collected through our website, affirmativetherapycollective.com. It applies to the website only — not to the protected health information we maintain as part of providing care, which is governed separately by our Notice of Privacy Practices.
This website is not a secure place to share health information. Please don’t send clinical details, diagnoses, or sensitive personal information through our contact form, screening form, or email. Once you’re a client, all clinical communication happens through our secure system (SimplePractice).
Information you give us. When you reach out through our contact form or complete our screening form, we may collect your name, email address, phone number, and whatever you choose to include in your message. We use this only to respond to you and, where appropriate, to help match you with a clinician.
Information collected automatically. Like most websites, ours automatically collects limited technical information such as your IP address, browser type, device, and pages visited. We use Google Analytics to understand how visitors use our site so we can improve it. Google Analytics sets cookies and processes this data under Google’s own privacy policy.
Cookies and your choices. Our site uses cookies for basic functionality and for analytics (Google Analytics). That relationship begins only when you and one of our clinicians formally agree to work together.
In a crisis, this site can’t help you. If you are in immediate danger or experiencing a mental health emergency, call or text 988 (Suicide & Crisis Lifeline) or call 911. Do not use this website or our contact form for urgent or emergency needs — we cannot monitor it in real time.
Where we provide care. Our clinicians are licensed in specific states and can only provide therapy to clients located in states where they hold a license. Visiting this site does not entitle anyone to services regardless of location.
